The key point
Some U.S. dental plans may provide out-of-network benefits for treatment outside the United States. The clinic cannot guarantee coverage, reimbursement, timing, or HSA/FSA eligibility. Your insurer or plan administrator is the authority for your benefits.
Before treatment
- Ask your insurer whether out-of-country dental services are eligible.
- Ask whether preauthorization or a pretreatment estimate is required.
- Confirm annual maximums, waiting periods, exclusions, and required records.
- Request written confirmation when possible.
Documents the clinic can provide
- Written treatment plan and itemized invoice
- Available X-rays and clinical records
- Procedure descriptions and appropriate CDT codes
- Proof of payment
Submitting the claim
Complete your insurer's claim form and attach the documents it requests. Keep copies of everything. If a claim is denied, ask the insurer for the exact reason and required appeal documents.
Important: Payment to the clinic remains the patient's responsibility regardless of insurance reimbursement.
